Optogenetic stimulation CheckMate 908 (NCT03130959), a phase 1b/2, open-label, sequential-arm study, investigates nivolumab (NIVO) and the combination of nivolumab (NIVO) and ipilimumab (IPI) in pediatric patients suffering from high-grade central nervous system malignancies.
In five cohorts, 166 patients received either NIVO 3mg/kg every two weeks (bi-weekly), or NIVO 3mg/kg plus IPI 1mg/kg every three weeks (four doses) followed by NIVO 3mg/kg administered every two weeks. The primary outcome measures were overall survival (OS) in newly diagnosed diffuse intrinsic pontine gliomas (DIPG) and progression-free survival (PFS) in other recurrent/progressive, or relapsed/resistant, central nervous system (CNS) cohorts. Secondary endpoints further included assessment of safety and other efficacy metrics. Pharmacokinetics and biomarker analyses were integrated into the exploratory endpoints.
In newly diagnosed DIPG, the median overall survival, calculated using an 80% confidence interval and reported on January 13, 2021, was 117 months (103-165) for the NIVO group and 108 months (91-158) for the NIVO+IPI group. NIVO treatment resulted in a median PFS (80% CI) of 17 (14-27) months in recurrent/progressive high-grade glioma, while NIVO+IPI yielded a median PFS of 13 (12-15) months. For relapsed/resistant medulloblastoma, NIVO yielded a PFS of 14 (12-14) months, and NIVO+IPI exhibited a PFS of 28 (15-45) months. Likewise, relapsed/resistant ependymoma patients treated with NIVO achieved a median PFS of 14 (14-26) months, compared to 46 (14-54) months with NIVO+IPI. The median progression-free survival (95% confidence interval) in patients with recurring/advancing central nervous system tumors was 12 months (11 to 13) and 16 months (13 to 35), respectively. Treatment-related adverse events of Grade 3/4 were observed in 141% of the patients in the NIVO group, and 272% of the patients in the NIVO+IPI group. Amongst the youngest and lowest-weight patients, NIVO and IPI first-dose trough concentrations were observed to be lower. Survival times were not affected by the programmed death-ligand 1 expression level detected in baseline tumor samples.
NIVOIPI's clinical benefits, compared to previous data, were not evident. Safety profiles, overall, were within manageable parameters, free from any new safety signals.

Seasonally and age-adjusted analysis of self-controlled case series data explored the temporal association between gout flares and venous thromboembolism. Following a gout flare, whether treated in primary care or a hospital, a 90-day period was deemed the exposure period. The duration was apportioned into three 30-day stretches. The baseline period was characterized by a two-year period preceding and following the exposure period's timeframe. A measure of the association between a gout flare and venous thromboembolism (VTE), employing adjusted incidence rate ratios (aIRR) with 95% confidence intervals (95%CI), was undertaken.
314 patients, conforming to the criteria of age 18, incident gout, and no prior history of venous thromboembolism or primary care anticoagulation before the pre-exposure period, were selected for inclusion in the study.
